Certificate of Residency

HACC certificates of residency may be obtained by appointment only. Call 218-5821 to schedule.

And one of the following documents are required to obtain a certificate of residency:

  • Drivers license
  • HACC ID#
  • Current lease or Real Estate Tax Bill or front page of most recent federal tax return or copy of most recent pay stub showing current address
  • W-2 for the most recent tax year
  • Copy of your most recent Local Tax Return or if you are a dependent of a resident, the front page of the most recent Federal 1040 Tax Return of the person who claimed you as a dependent.
  • Copy of your most recent pay stub and a pay stub from 6 months ago showing the same address.
  • Registered property (such as a bank account or car)
  • Voter registration
  • Military orders showing assignment to an active duty station within the SMSD boundaries.
  • Copy of most recent utility bill and a utility bill from 6 months ago showing an address within the SMSD boundaries.
